Ammonia refrigeration is one of the older types of refrigeration that is still used today. The
ammonia is the chemical that is used to absorb the heat from one area, and bring it to another
area to dissipate. Concentrated ammonia is much colder than typical room temperature, which
makes it an excellent choice for keeping things cool.
A catastrophic ammonia released at the Fernie Memorial Arena on Oct. 17, 2017 has resulted
in three workers lost their lives while performing maintenance on ice-making equipment at the

WorkSafeBC is preparing an incident investigation report (IIR) of the incident. You are one of
the investigation team and you need to prepare the report.
a) In your report, apply the five (5) factors in sequence that lead to the incident as
proposed by Domino Theory. The incident animation may guide you to prepare the
report.

to obey. Justify your answer.

b) Analyse the hazard potential of ammonia in (a) that correspond to the incident
happened in Fernie Memorial Arena in Question 1. Justify your answer.

c) Evaluate the concentration of ammonia released during the second mechanical failure
in the incident animation that leads to the death of the three workers with the exposure
concentration limit. State all your assumptions.

d) Recommend the mitigation technique that can minimize the severity impact to human.
