JOINT AFFIDAVIT

We, Spouses _______________ and ______________, all of

legal age, Filipinos, and residents of _______________, Zamboanga
City, after having been sworn in accordance with law, hereby
depose and say:

That we are the parents of ______________, who was born on

_____________, in Zamboanga City, Philippines;

That at the time our said daughter was born, we were not yet
married, because I, ____________ as her mother, was a seventeen
(17) year old minor on account of which I wasn’t qualified to
contract marriage;

That due to the said circumstance, when the birth of our

said daughter was registered in the Office of the Civil Registrar
of Zamboanga City, her name was recorded as _____________, as
shown by her CERTIFICATE OF LIVE BIRTH, a copy of which is hereto
attached as Annex “A”;

That subsequent to the birth of our said daughter, we

formalized our marital union by a marriage celebrated on
___________, as shown by our CERTIFICATE OF MARRIAGE, a copy of
which is hereto attached as Annex “B”;

That in her school and other official records, we and our

said daughter have used the name __________________, i.e., with
the surname of father’s surname;

That the names _______________ and ___________________ refer

to our said daughter as one and the same person;

That we are executing this affidavit to attest to the truth

of the foregoing narration of facts for all the legal purposes
that this may serve.
