I, Ranjit Saha worked in Haldia Petrochemicals Limited from 01/07/2010 to 23/07/2013 and my PF
account number was WB/PRB/25181/2128. I have joined Brahmaputra Cracker and Polymer Limited on
31/07/2010.

When I applied online for transfer of my PF amount through present employer i.e BCPL from my
previous establishment i.e Haldia Petrochemicals account number (WB/PRB/25181/2128) it shows that
my date of birth is not matching with the previous one i.e as per record of previous establishment. After
checking it in my present PF e- passbook. It is found that the date of birth is correct in the records of
my present organization which is 28/09/1985.

I will be obliged if you correct my date of birth (28/09/1985) in your record, so that I can transfer my
Previous PF amount to my present account . My pan card Xerox copy is also attached with this
application for supporting document.
