A is found on the next page of the guidelines in this MS Word file.
1. Acquire the soft copy of the e- copy from the authorized yeabook staff at the Student
Publication Office at the Office of the Student Affairs (OSA).
2. Strictly, completed soft copy of the e-copy must be submitted to the yearbook staff on/
before deadline of submission, the schedule of which will be announced/posted at the official
social media pages of the school or at the school bulletin boards. Your respective class mayors
(or any class representatives) will be responsible for the collection of your data copies and they
will be sorting them out in one MS Word file per course and section (NOTE TO MAYORS: Class
names should be arranged alphabetically). The class mayors (or any class representatives) will
also be responsible for the submission of said completed data to the yearbook staff.
3. It is also of high importance that a graduating student will be THE ONE putting the data
entries in the yearbook data e- copies. This is for in any regards that the data will be
personalized by the owner himself/herself and erroneous/wrong entries will be avoided as far as
his/her data is concerned.
1. Kindly refer to Appendix A the data e- copy that should be filled out with data entries of the
graduating students. The provided data e- copy should follow the typing specs: 12, font size;
Calibri, font style.
2. Upon filling out, DO NOT put space after “>” on each of the data entries required. Format of
the name entry should be: Surname, First Name, Middle Name

3. Mother’s name should be the legal name she uses now on public documents, filling out of
forms, etc.
4. For students whose address is in North Cotabato, kindly follow the correct format below.

5. You can input as much as 5 organizations and 5 awards on your data e-copy.
