JOINT AFFIDAVIT

WE, ARISH JEAN EUGENIO and JALIBETH A. FONTANILLA, married and

single respectively, of legal ages, Filipinos, and both presently residing at Purok 5,
Andres Bonifacio, Diffun, Quirino having been sworn in accordance with the law hereby
depose and state that;

1. That we personally know AIRISH ANNE TUMBAGA-UGALE as an acquaintance;

2. THAT we know for a fact that she is an owner of restaurant located at Andres
Bonifacio, Diffun, Quirino registered as RAIN’s KITCHEN;

3. That sometime in April 2020, she informed us that she submitted our names in her
application to DOLE CAMP so we can have at least something to claim this pandemic,
however, with the condition that she will have a share should we successfully claim it;

4. That she went to JALIBETH FONTANILLA’S house and ask us as to sign a blank
payroll where seven (7) names were listed including us;

5. Further, she message us through messenger to remind us “pag may tumawag

tandaan mo rains kitchen kau work 10,350 sweldo waitress kau ni abbet”
“airish ugale and may ari”…. “365/day”;

6. That on April 28, 2020 after receiving a message from ML Kwarta Padala that we can
already claim our money, we immediately rushed to the nearest branch and
successfully claimed it;

7. That on the following day, AIRISH ANNE TUMBAGA-UGALE asked us if we

already claimed the benefits from DOLE CAMP and we answered in the positive;

8. That she informed us that he will instruct, DENNIS, his husband to claim their share,
however, we did not come to an agreement as to the specific amount;

9. That on the following day, AIRISH ANNE TUMBAGA-UGALE’s mother,

PUNONG BARANGAY AIDA TUMBAGA confronted us and urged us to just return
the SAP, but instead,we decided that the DOLE CAMP benefits be returned since we
know that we are not legally entitled to it;

10. That since we are in good faith and determined to return the DOLE CAMP
5,000php benefits which we are not legally entitled thereof, we now have returned it
through PESO DIFFUN QUIRINO, which is hereto attached as Annexes “A” & “B”;

11. That we are not employees of Rain’s Kitchen nor have received the amount of
10,350.00 php as stated in the payroll;

12. That we are executing this affidavit to prove that we have returned the 5,000
benefits from DOLE because we already received SAP from the DSWD and for all legal
intents and purposes it may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 21st day of May
2020, at Diffun, Quirino.
