You are a Pharmacist in a small, rural, independently owned community pharmacy. Ms. Loren Riley is
a 63 year old white female. She is weel known to you and the rest of the staff at the pharmacy as a
patient who is somewhat "difficult", often complains about the price of her medications, and is
always in a big hurry. She comes in one monday morning and ask the cashier for some eucalytus oil.
You overhear her request and quickly walk out to find out what current problem she is having. She
exasperatedly tells you that she was a wart on her foot, and that when she had a wart in her teens,
her mother had put eucalyptus oil on it every day for a few weeks and it had cleared up beautifully.

You ask Ms. Riley to step into the private patient counseling room so that you can speak with her
further. She is irritated and tells you that she just wants a little bottle of the oil, and she has a lunch
date. You insist, however, and she agrees to talk with you briefly.

In the counseling room, Ms. Riley tells you that about 2 weeks ago she first noticed what felt like a
"pebble" in her shoe during her 2 mile walk, and that the place on her foot has worsened somewhat
since then. She describes it as "uncomfortable", rather than painful. It has, however, kept her from
her normal walking routine but not from attending water aerobics at the local aquatics center twice
weekly. When asked, she reluctantly takes off her shoe to show you the bottom of her foot. You see
an eraser sized lesion on the ball of her foot. It is not noticeably raised and interrupts the normal
whorls of the foot.

You agree with Ms. Riley that she has a plantar wart. "Well, I told you that," she replies. " That's why
I asked for the eucalyptus. Do you have any or not?" You explain that you need to ask just a few
more questions to determine if eucalyptus oil is a good choise for her. You learn that Ms. Riley was
widowed about 5 years ago, and now lives with her daughter and son-in-law. She has never used
any tobacco products and has a glass of wine when she goes to dinner with her "gentleman friend"
about twice a month. She is a retired school librarian and lives off her husband's pension and social
security benefits. She proudly tells you that she is very healthy, and that her "Pressure" is under
good control with only two prescription medications, but she cannot tell you the names of the
medications. "and it's a good thing," she says, "as expensive as that one is!" (At this point, you ask
your technician to give you a printour of the patient's prescription profile from the beginning of the
year, and check Ms. Riley's blood pressure. It is 124/82). She also says that she takes a baby aspirin
every day, "just in case," and a couple of Tylenol Arthritis when she needs it (a couple of times a
month). When questioned about herbal preparations, she tells you that she has been taking black
cohosh for about a month, on the advice of her physician, to help with her hot flashes and mood
swings, which she has been putting up with for years, but for which she refuses to take any
"medicine" (i.e. prescription drugs). She has noticed a significant improvement and is planning on
continuing to take it.

Your technician brings you Ms. Riley's prescription profile (Case Table 79-1).

Ms. Riley is now obviously ready to go. She ask what she should do if she should not use the
eucalyptus oil for her wart. She just wants it to go away as quickly as possible.
