RE: Request for Deferment of Physical Examination

TO WHOM IT MAY CONCERN:

Greetings!

I am Francheska Andrea C. Taylan, an incoming ____ year BS Physical Therapy student at


_______. I am a candidate for Clinical Internship for the incoming Academic Year 2020-2021
and one of the pre-conditions of my enrollment as an intern is to undergo a physical
examination at __________. However, due to the COVID-19 crisis our country is suffering at the
moment, I was forced to go home to the province prior to the Luzon-wide Lockdown /
Community Quarantine on __________.

That despite the relaxation of the National Government’s quarantine measures, the Provincial
Government of Ilocos Norte has enacted certain protocols to minimize, if not, prevent the
transmission of the Covid-19 virus in the locality. One of which is the mandatory 14-day
quarantine of returning Ilocos Norte residents who have travel history or exposure to places
categorized by IATF as high-risk area.

Considering that the physical examination requires me to travel to Metro Manila which is
categorized as a high-risk area, I will have to undergo the mandatory 14-day quarantine
imposed by our local government unit upon my return to Ilocos Norte. Worse, all expenses
relative to the said quarantine will be personally shouldered by the person undergoing
quarantine, and not subsidized by the local government.

In this connection, I would like to humbly request from your good office the deferment of my
physical examination for at least three months or until our actual clinical internship in order to
avoid any unnecessary delay and financial burdens on my part.

With the extraordinary situation right now, I am confident that you will exercise your wise and
sound discretion. Hoping for your favorable response on my request.
