In the jasmine room there is a nurse who will do a physical examination to Ms.

firda to find out if


there are abnormalities in the patient.

Nurse: Good morning My name is Nurse Indang. May I know your name?

Patient: Good morning Nurse. My name is Bela.

Nurse: Ok Miss Bela. How are you feeling today?

Patient: I feel a headache and my neck hurts when I swallow something.

Nurse: Alright. I will check your head and neck to see if there are any abnormalities. Are you ready?

Patient: Of course Nurse.

Then the nurse prepares the tools needed to examine the patient.

Nurse: First, I will check your head . There are no bumps on your head and no scars.

Patient: Alright Nurse.

Nurse: Do you feel pain in your head? (The nurse palpates the patient's head)

Patient: Yes. I feel pain when you palpate Nurse.

Nurse: Where do you feel pain?

Patient: On the back of my head Nurse.

Nurse: Alright. Now I will check your neck. Are you ready Miss Bela?

Patient: Yes Nurse.

Nurse: Your neck looks symmetrical.It’s normal. Now, can you try swallowing?

Patient: (the patient swallowed and then the nurse touches the patient's neck).

Nurse: There is an enlargement of the thyroid gland on your neck. This means there are
abnormalities. Do you feel pain?

Patient: Yes, I feel pain Nurse.

Nurse: If I give a scale of 1-10, how much pain do you feel?

Patient: The pain scale that I feel is 5.

Nurse: Ok I will report your complaint to the doctor.

Patient: Thank you so much Nurse.

Nurse: Now I will do auscultation using a stethoscope.

Patient: Alright Nurse.

Nurse: There is no systolic noise on your neck.

Patient: Alright Nurse.

Nurse: After I checked, there are some abnormalities that I found and I will report it to the doctor.

Patient: Thank You so much nurse.


Nurse: OK. Now I will back to nurse office if you need help you can call me. Get well soon Miss Bela.

Patient: Yes Nurse. Thank You.
