ONLINE SPECIAL EDUCATION SESSIONS AGREEMENT FORM

In light of the ongoing situation in our country in its fight against COVID19,
TW Community Enablers has initiated holding online special education
sessions to help students and parents how to continue the child’s education
amidst the quarantine. Although face to face classes are still preferrable,
our situation has pushed us to discover creative ways to continue in our
passion to educate your children. The use of online technology has been
studied in recent researches and has shown promise in ensuring that the
teaching -learning process is facilitated.

Online special education sessions will only be done during the period of
the community quarantine set by the government due to the Covid-19
threat, that is from March 18 to April 30, 2020. If the government
decides to lift the community quarantine and orders businesses to
resume, Online special education sessions will no longer be provided.
On the other hand, if the government extends the community quarantine
period, provision of Online special education sessions may likewise be
continued based on the directive of the government.

This agreement is to be entered into by TW Community Enablers, Inc., the


licensed and registered therapy or education professional and the parent/
guardian of the child.

• The teachers agrees to provide therapy sessions online via two


methods:


1)direct intervention to the child if the child is able to sit still and focus
on the laptop;
2) parent coaching/guidance regarding issues that are of immediate
concern


• The teachers agrees to provide 40 minutes to one hour sessions for


the child / parent to ensure that methods or program is understood. 


• The teachers will send requirements such as materials to be


prepared, online links, printed material one day in advance for the
parents to download and print. 


• After the session, the teachers shall provide session notes to be


emailed to the parents and to the twce.setep@gmail.com with the
following heading: 

1) subject heading: Notes date(yyyy/mm/dd)_student’s last name/first
name_teacher’s last name)example: Notes 2020/03/23_Reyes/
Bubbles_Paguio

2) subject body: time started and time ended; participants; notes



3) send a screenshot of start time, middle, end time

4) notes will be provided to the parents in PDF format

B. Parents or guardians agree to the following:

• 1)  Participate in a 40 minute to one hour session with the teacher; 


• 2)  Have 1 adult in the designated online session area to ensure the


child is focused on the session; or have 2 adults at the designated
area wherein 1 adult is engaged with the teacher and another with
the child.


• 3)  Prepare a conducive work area where the child and adult will have
their session with the teacher, preferably well lit and ventilated with a
cleared out area to work in. 


• 4)  Download and prepare materials sent by teacher ahead of time; 


• 5)  Collaborate with the teacher as to the most pressing needs of the


client. Due to the limitation of our medium, teacher and parent may
need to prioritise areas that will receive intervention first. 


• 6)  Do return demonstrations with the teacher/student or send 1-2


minute videos of the implementation of the activity for the teacher to
comment on and give suggestions to.


C. Furthermore, parent agrees to payment of the fees stipulated at the end


of this form. Remittance of payment may be in the form of:

a) Online banking through: Bank of the Philippine Islands

An acknowledgement will be sent via email once deposit slip is received. b)


Payment may also be made when sessions resume at the facility.

C. All sessions will be charged with the same rate as center-based


sessions, specifically for this time of community quarantine. Re-evaluation
of the entire Online Special Education Sessions delivery method will take
place once quarantine is over and if the parents want to continue with this
method.

D. Parents may terminate future sessions with the agreement of their


teacher if they feel the needs of their child are not being met.

I agree to the terms of this Online Special Education Sessions Agreement
