Library Instruction Evaluation Plan

In addition to the evaluation from the collaborative lesson plan [included below], I would want to
have a brief reflection meeting with the collaborative teacher. We would talk through some of
the questions listed below, but I would also want their feedback on the following questions:
1. How well did the collaborative lesson meet your curricular needs?
2. What further resources and support do students need on this topic?
3. What specifically was helpful (or unhelpful) about collaborating on this unit? [In
particular, I would want to know if this collaboration lessened their workload - to foster
collaboration with teachers, it’s important that we not add to their workload.]

I would also want to give the students a brief survey, likely through Google Forms to make it
easier to track the data. This is a general survey that I could use for any collaborative lesson or
unit. In addition to a couple questions about what day they worked with me and what the
lesson’s topic was, I would include the following questions:
1. After the lesson, how well do you understand the topic on a scale of 1-5? 1=I’m still
totally lost. 3=I still have questions but I know more than I did. 5=I understand everything
we worked on.
2. What was your favorite part of the lesson?
3. What questions do you still have?
4. What resources (books, websites, etc.) could the librarian provide to help you further
your understanding of the topic?

For myself, I would also want to think about: How could this lesson/unit be adapted for other
content areas? How can I provide support for the content teacher and students when they
review this topic?
