AFFIDAVIT OF SOLE HEIRSHIP

I, , 22 years old, single, Filipino citizen and a resident of Brgy. Tigum,

Pavia, Iloilo, after having been duly sworn to in accordance with law do
hereby depose and say:

1. That I am the only heir of herein as “Annex A” is a copy of his

Certificate of Death;

2. That as evidenced of my legal relation to the deceased, a copy of

my Certificate of Live Birth is attached herein as “Annex B”;

3. That I caused the construction of a small farm house in Lot No.

which is owned by the deceased, situated at Brgy. Tigum, Pavia, Iloilo.
Attached herein as “Annex C” is a copy of Transfer Certificate of Title.

4. That I am executing this affidavit in good faith to attest to the truth

that there is no other heir or heirs of the deceased and for the purpose of
applying for the issuance and installation of an electric meter and for
whatever legal purpose it may serve.
