AFFIDAVIT OF UNDERTAKING I,
MR. ROLAND A. ALJIBE, Filipino, of legal age, married with residence address at Brgy. Culasian,
Capoocan, Leyte, after having been subscribed to an oath, depose and say: 1. That I am the incumbent
CHAIRMAN OF THE BOARD OF DIRECTORS of CAPOOCAN UNITED FISHERFOLK MULTI-PURPOSE
COOPERATIVE, a duly registered cooperative with the Cooperative Development Authority under
Registration No. 9520-08001302. 2. That pursuant to CDA Memorandum Circular No. 2015-08, the
cooperative must be compliant with all the requirements in order to receive a Certificate of Compliance.
The deadline of submission of the requirements is on April 30, 2016. 3. That the Annual General
Assembly C.Y. 2016 of the cooperative is scheduled this May 15, 2016, therefore, the cooperative cannot
submit the minutes of the Annual General Assembly of the current year this coming April 30, 2016. 4.
That I, ROLAND A. ALJIBE, assure and take full responsibility on the submission of the said lacking
document for the filing of Certificate of Compliance seven (7) days after the aforementioned date of the
CUFMPC Annual General Assembly C.Y. 2016. In witness hereof, I hereby set my hand this ____ th day of
