UNIVERSITY OF LA SALETTE, INC Santiago City, Philippines ENROLLMENT PROCEDURES THROUGH ONLINE SIAS PLATFORM AND OTHER MODALITIES For Old Students 1. Issue enrolment form through the following schemes: a. Registrar's Office issue Enrolment Form for those who will come to enroll personally. b. The Dean facilitates the issuance of Enrolment Form in an editable pdf format through FB Messenger or e-mail. ¢. Student enroll directly in the ONLINE SIAS (the step-by-step procedure will be provided) 2. Provision for academic advising a. Through the SIAS Platform: The Dean pre-enlist the courses that the student will enroll b. Through FB Messenger/E-mail: The Dean sends Student Evaluation of Unfinished Subjects along with an e-copy of Enrolment Form. In the Student Evaluation of Unfinished Subjects, it should specify the courses to be enrolled by the student. 3. Approval of course load: When confirming the student's enrolment, the following condition should be considered: a. If the student has lacking health examination requirements, the approval of his/her enrolment is conditional. He/she must comply with the requirements within June 2020. For the First Semester, the requirements must be complied with in September 2020. b. Ifthe student belongs to the list of medically high-risk students, clearance must be secured from the Director of Health Services, ‘The course load shall only be approved upon receipt of the consent from the Director of Health Services. 3a SIAS Platform: The Dean approves the enrolment of the student. 3.2 FB Messenger/E-mail The Dean forward printed copy of approved Enrolment Form at the Registrar's Office for encoding 4. Encoding at the Registrar's Office a. SIAS Platform: The Registrar's Office follow the procedure as prescribed in the SIAS platform FB Messenger/E-mail: ‘The Registrar's Office encodes the courses based on the submitted copy. If the student has existing unpaid fees ¢.1 The Dean informs the student of their outstanding balance 2 Students pay the outstanding balance through the bank account. The designated bank are as follows: Security Bank Corporation- Santiago City Branch (for undergraduate) Account Name: —_—_ University of La Salette, Inc Account Number: 0691-01359-2001 Security Bank Corporation-Santiago City Branch (for graduate school) AccountName : —_—_ University of La Salette, Ine Account Number: 0691-01359-2002 OR Land Bank of the Philippines — Santiago City Branch (both) Account Name: University of La Salette, Inc Account Number: 1362002555 t. 3 The student sends the deposit slip with his/her name written on the deposit slip through this e-mail: accounting@uls.edu.ph ¢4 — The Accounting Office validates the payment and replies e- mail by sending a copy of the Official Receipt. ¢.5 The Accounting Office furnishes the Registrar's Office a copy of the list of students who made bank payment (daily) as a basis for the encoding of enrolment or approval of enrolment through the ONLINE SIAS platform. c.6 The Registrar's Office sends the encoded Enrolment Form at the Accounting Office for the assessment of fees. 7 The Accounting Office forward “assessed” Enrolment Form and Assessment Form to the Dean of the student. ¢.8 The Dean informs the student of their fees and returns the documents at the Accounting Office, c9 Student pays through the designated bank accounts, and send deposit slip with their name through e-mail 10 Upon verification of payment, the Accounting Office stamps the Enrolment Form with “enrolled” and forward the Enrolment Form at the Registrar's Office for validation, 5. Validation of Enrolment The Registrar's Office print the Enrolment Validation Form and forward the Student Copy and Dean’s Copy of the Enrolment Validation Form to the concern Dean of the student ENROLMENT PROCEDURE (For New Student) ‘The new student applies for enrolment through any of the following schemes: a. For enrolment through the SIAS Platform, follow these steps aa Click uls.edu.ph a2 Click “connect” then “online sias” a3 Click “admission” ag In the form click “new application” as Fill-up the Admission Form. It is important to take note of the issued control number a6 For the item on “choose exam date”, indicate the current date az Click the box on “confirm that the information is correct” a8 Click “save” b, For enrolment through FB Messenger/E-mail : ba The applicant accomplishes the General Information Sheet bo ‘The Dean submit the accomplished General Information Sheet at the Human Development Center for the notification of payment of Registration Fee 2, The Human Development Center download the accomplished application of new students from the ONLINE SIAS and notifies the applicant to pay a registration fee of P1,000 to any of the following bank accounts of the university: Security Bank Corporation- Santiago City Branch (for undergraduate) Account Name f University of La Salette, Inc Account Number: 0691-01359-2001 Security Bank Corporation-Santiago City Branch (for graduate school) Account Name z University of La Salette, Inc Account Number 0691-01359-2002 OR Land Bank of the Philippines — Santiago City Branch (both) Account Name 3 University of La Salette, Inc Account Number; 1362002555 ‘The Human Development Center shalll include in the notification that deposit slip with the name of the student be sent through e-mail at accounting@uls.edu, ph 3. The Human Development Center forward the application of new student downloaded form the ONLINE SIAS and General Information Sheet at the Accounting Office 4. The Accounting Office validate bank payment and send a confirmation e-mail with an e-copy of their official receipt 5: The Accounting Office forward application of new student and General Information Sheet with paid Registration Fee at the Registrar's Office 6. The Registrar's Office notifies the applicant of their SIAS Account Number and Password through text message. 7. Students can now enroll using the SIAS Platform Submitted by: Approved by: MR. PICIO, MS, PhD Chairpersoyi, Committee on Admission University Presidgnt Procedure of online enrolment through the ONLINE SIAS Visit the website www.uls.edu.ph Click “CONNECT” then “ONLINE SIAS.” Click “STUDENT” Log-in your USER ID and PASSWORD. The USER ID is your STUDENT NUMBER, PERE Note. In case you forgot your password, send e-mail to inquirie@uls.edu.ph At the ONLINE SIAS menu, click “TRANSACTION.” Click ENROLMENT”. Courses to be enrolled will be displayed 7. Click “AUTO”. Courses with the class schedule will be displayed in a: oe ss column, Note. Ifno schedule appears in any of the courses, click the said course and select among the schedule listed in the schedule area. 8. When the schedule of courses is finalized, click “ASSESS”, The assessment of fees will appear. 9. Send deposit slip with your complete name through e-mail at accounting@uls.edu.ph 11, Wait for confirmation through e-mail with your official receipt 12, To check if you are official enrolled, log-in to your ONLINE SIAS account 13. Click “REPORTS” then “ENROLLED SUBJECTS” and Note: A remark “OFFICIALLY ENROLLED” appears, 14, To print your copy, click “PDF”
