CORPORATE HEALTH SERVICES (CHS)

CHS is a Blue Cross Blue Shield of Arizona (BCBSAZ) product that provides large, self- insured
groups access to the BCBSAZ provider networks. Claims processing, investigation of other party
liability, verification of eligibility, benefits, claim payments, claim status, HIPAA compliance and
the Explanation of Benefits (EOB) are handled by the Third Party Administrator (TPA) or
employer group.

This listing contains all approved CHS group numbers. All claims submissions are required to be
billed with the CHS group number.
