DATA PRIVACY CONSENT FORM

TERMS OF AGREEMENT FOR APPLICANTS FOR ADMISSION

Please check the appropriate statement:

[ ] I am a student-applicant who is 18 or over 18 years of age.

[ ] I/ We am/are the parent/s or legal guardian of a student-applicant who is a minor (below 18 years
old).

By signing below, I/We warrant that I/we have read, understood all the provisions and agreed with its
full implementation:

1. Republic Act No. 10173, or the Data Privacy Act of 2012, and other relevant Philippine laws
apply to the collection and processing of my/ applicant’s personal data.
2. I/We understand that by applying for admission to St. Scholastica's College, I/we give permission
to the institution to collect, use and further process information on personal circumstances and
directory information such as – but not limited to – physical addresses, landline/mobile
numbers, email address, family data/history, academic information/background, disciplinary
record, medical record and/or other significant demographic profile.
3. I/We am/are aware that said personal information will be accessed and used by the Admissions
personnel for the purpose of carrying out their contractual duties and by other school personnel
who are involved in the evaluation of application for admission.
4. By providing personal information to the Admission Office, I/we am/are confirming that the data
is true and correct. I/We understand that St. Scholastica’s College Manila reserves the right to
revise any decision made on the basis of the information I/we provide should the information be
found to be untrue and incorrect.
5. I/We also understand that the photocopy of the personal documents that I/we will submit in
relation to the application will be properly disposed by the institution if not accepted or if I/we
decide not to pursue the application. However, once accepted and upon enrollment I/we
submit the original copy of the needed personal documents. Documents of enrolled applicants
will be forwarded to the Office of the Registrar.
6. Any issue that may arise in connection with the processing of the personal information will be
settled amicably with the institution before resorting to the appropriate arbitration or court
proceedings within Philippine jurisdiction.
