3. I understand that submission of the appropriate academic credentials is a pre-requisite to enrolling at St.
Paul University Quezon City;

8. I agree to indemnify and hold St. Paul University Quezon City, its trustees, administrators, officers,
employees and agents free and harmless from any and all liabilities, suits, claims, demands, damages,
judgments, costs, fines, penalties, interest, and expenses (including all professional fees and expenses
thereof) now or hereafter arising out of and in connection with the performance of my obligations
hereunder;

9. I consent to providing and to the processing of my personal and sensitive personal information as provided
by law in order to fulfil my duties, obligations and responsibilities under this Undertaking;

10. I confirm that I sign this document free and with full understanding of its content and consequences.

CONSENT FORM

I hereby certify that all I have indicated herein are true and correct. Any misinformation shall
serve as ground for the nullification of my child's enrollment at SPUQC. If my child is accepted as a
pupil/student in this University, I pledge that my child will fully abide the school's policies and
regulations.

I consent to the collection, recording, organization, storage, updating and modification, retrieval,
consultation, use, consolidating, blocking, erasure or destruction (if necessary) and, in general, the
processing of the personal information I have provided SPUQC in this enrollment form, and other
enrollment-related forms pertaining to the status my of child at the University, to serve the
educational purposes of the University, and to enable it to comply with its legal and reportorial
obligations under the law and regulations, and to enable it to facilitate, monitor and improve
educational services towards its student, the government and stakeholders.

I agree that the personal information I have provided in the enrollment and other enrollment-
related forms, may be processed, disclosed, transferred or shared to enable the school to comply
with its obligation, including, but not limited to, submission of periodic reports to government
agencies having authority over the school such as, but not limited to, the Department of
Education, Bureau of Foreign Affairs, Technical Education and Skills Development Authority, etc. or
private agencies such as, but not limited to, Philippine Accrediting Association of Schools, Colleges,
& Universities (PAASCU), International Organization for Standardization (ISO), Philippine
Association of Colleges and Universities Commission on Accreditation (PACUCOA), Catholic
Educational Association of the Philippines (CEAP), arising from or in connection with my child's
relationship to the school.

I understand that the University, the Congregation and other Congregation schools will not use my
child's personal information for commercial purposes. The information herein shall be treated as
confidential in compliance with the Data Privacy Act of 2012.
