DEED OF UNDERTAKING

Agnawe, Guba Cebu City (Address), hereinafter referred to as
“STUDENT”.
Witnesseth—
WHEREAS, the parties execute this Deed of Undertaking in compliance with Joint
Memorandum Circular No. 2021-004 issued by CHED.
NOW, THEREFORE, for and in consideration of the foregoing premises, the Parties
hereby undertake, as follows:
1. LIMITED FACE TO FACE. As expressly allowed by CHED, DOH and the IATF, SWU
shall allow a limited face to face classes for its 3rd year students of the BS Pharmacy
Program. They will be allowed to enter the campus for purposes of attending lecture,
laboratory classes, and to take examinations.
2. UNDERTAKINGS OF SWU:
a. Promulgate protocols and procedures for the clerkship in consonance with the
health guidelines as imposed by the Department of Health (DOH), Commission on
Higher Education (CHED), and the Local Government Unit (LGU) through the
College of Pharmacy;
b. Implement stringent observance of such health protocols and closely monitor
compliance thereof by the STUDENT through the Assistant Dean;
c. Immediately investigate and correct even the slightest notice of any disregard of
such health protocols through the Dean;
d. Only allow students to attend the face-to-face classes who have complied with
Philhealth coverage requirement or a similar health insurance coverage;
e. Ensure that only vaccinated students and teaching and non-teaching personnel will
be allowed to participate in the limited face to face classes.
f. Ensure appropriate and adequate medical management through SWU Medical
Center for those STUDENT with moderate to severe symptoms of COVID-19
Infection.

b. Comply with all requirements stipulated in the CHED-DOH Joint Memorandum
Circular No. 2021-004.
c. Stay at home and report to the faculty any possible exposure or manifestation of
symptoms of COVID19;
d. Present his/her vaccination card to the faculty;
e. Treat other students, teachers, and school personnel as potentially infectious, thus,
must at all times, observe strict health protocols and physical distancing.
4. ADMISSIONS:
a. The Student understands that he/she can be removed and will not be allowed to
continue with the limited face-to-face classes if he/she will violate or shall commit
any infraction that will endanger himself/herself or others students, teachers, or
other workers of SWU.
b. The Student is fully aware of the possibility that infection may happen while within
campus and has agreed to assume such risk.
c. The Student is willingly and voluntarily joining the limited face-to-face classes,
without any inducement or intimidation from the College of Pharmacy or any of
SWU’s officers, faculty, and representatives.
5. WAIVER:
a. In view of the foregoing, should there be any incident during the limited face-to-face
classes that the Student will be exposed to COVID19, he/she will not hold SWU, its
faculty, affiliate companies, administrators, directors, officers, assigns, affiliates,
employees and representatives, LEGALLY RESPONSIBLE and shall take upon
himself/herself any and all consequences and liabilities as a result of such exposure.
b. The Student hereby WAIVES, RELEASES, QUITS ANY CLAIM, DISCHARGES,
and shall indemnify and render free and harmless, SWU, its faculty, affiliate
companies, administrators, directors, officers, assigns, affiliates, employees and
representatives, from any and all LEGAL liabilities, claims, demands, actions, and
causes of action, whatsoever arising out of or by reason of his/her decision to join
the limited face-to-face classes.
IN WITNESS WHEREOF, the parties have signed this Joint Undertaking on _________,
at Cebu City, Philippines
