I, MELCHOR RAYMUNDO MIRANDA, of legal age, Filipino Citizen,

Married and with residence address at No. 35 B. Rivera Street, Tinajeros,
Malabon City, Metro Manila, after having been duly sworn to in accordance
with law, do hereby depose and state:

1. That in my Certificate of Live Birth issued by the Civil Registry of

the City of Manila, my middle name was not entered and indicated;

2. That my true and correct middle name is Raymundo which is the

true and correct Surname or Family Name of my Mother, Avelina Dela Cruz
Raymundo;

4. I execute this Affidavit to attest to the truth of the foregoing facts

and for whatever legal intents and purposes this may serve.
