To successfully complete my QCHP medical license within my probation period.

1. Submitted all the necessary documents pertaining to my licensing requirements such as


Good Standing Certificate from Nursing Board, attested TOR, diplomas and credentials.
2. Medical testing appointment has been done on July 12, 2017 @ 09:00H.
3. Finger printing appointment has been done on July 17, 2017 @7.30 AM
4. Residency permit issued on July 23, 2017 and submitted a copy to licensing department.
5. Provisional medical license received on July 26, 2017.
6. BLS training done on September 24, 2017 at SMRC
7. ILS training booked on December 29, 2017 at HITC
8. BLSO training booked on February 03, 2017 at HITC
9. Official QCHP medical license approved and received on August 22, 2017.

Milestone: 04/10/2017- QCHP Medical license has been approved and received earlier than the
expected target date.

To use appropriate resources such as clinical eLearning’s, core orientation, certification programs and
trainings provided by Women's CMG to complete the OPC Orientation passport during my probation
period.
1. Accessed clinical eLearning‘s on LMS, reviewed and familiarized corporate eLearning
activities/modules.
2. Attended mandatory orientations such as:

Clinical Nurse Leaders and attended lectures given by Women’s CMG doctors.
8. Visited and familiarizing the location of offices and their services they provide such as HR, Relocation,
Housing and etc.
9. Attended the Perinatal Education Orientation Program on 08-08-10/2017.

On a continuous process, to be involve in the clinical activation and opening of the GYN/ IVF Clinic.
1. Attended and participated in Reproductive Medicine weekly divisional meeting.
2. Attended and participated GYN clinic activation team.
3. Participating on the ongoing research statistic for fertility centers/ clinics within GCC, Lebanon
and Jordan countries.
4. Provides update and feedback on the Fertility clinics statistic to the Reproductive Medicine
team for business intelligence purposes.

On a continuous basis to be acquainted with the protocols and guidelines of GYN/ Reproductive
Medicine sets by the Women's Clinical Management Group.

3. Attended training, lectures and conference for knowledge and practice update.
