orientation phase. I will collaborate with the various members of the multidisciplinary teams to help
ensure that the clinic is providing patient centered care and help to identify and contribute to the
development of clinical pathways, education materials, policies and guideline for each new clinic. I
will also report any workflow deficiencies and/ or solutions to my leaders to help improve the care
provided in the OB/GYN clinic.

I will help improve the quality of care provided in the OB/GYN clinic by identifying any safety and
quality issues to my leadership. By the end of December of 2017, I will identify and participate in any
quality projects initiated in the clinic and assist in the collection and analyzing of clinical data, and by
performing chart and physical audits of my peers on a monthly basis or more frequently if identified
by leadership.

By the end of December 2017, I will attend 18 Thursday educational sessions in my department and at
least one conference for the year. I will also help identify those educational topics that are relevant to
my role and clinical practice and communicate these topic to the educational department and my

By the end of December 2017, I will assist in the professional development of all newly on boarded
staff members assigned to me by assisting them in the completion of their orientation passport and by
ensuring that all new staff members complete their unit orientation and passport within 8 weeks of
starting on the unit. I will also review the orientation passport for my clinical role and report any
deficiencies to the education department and my leadership.
