JOINT AFFIDAVIT OF TWO (2)

DISINTERESTED PERSONS

WE, ELINA A. GUZMAN and REMEDIOS P. LUMONTAD, both of legal age,

married and residents of Cabinuangan, New Bataan, Compostela Valley Province,
Philippines, after having been duly sworn to in accordance with law, do hereby depose
and say, THAT:

We personally know ZALDEE ENTERINA ALVAREZ who was born on 13 May

1981 at Matina Crossing, Davao City to Spouses Marilou Bongolto Enterina and
Leonardo R. Alvarez;

We know him because he is our friend for a long time and neighbor;

In his Certificate of Live Birth issued by the Philippine Statistics Authority (PSA),
his mother’s first name was as erroneously entered as “MARIA LUZ” instead of
“MARILOU” which is her true and correct first name as appearing in the latter’s
Certificate of Live Birth issued by the same office under Registry No. 2015-120;

The names “MARIA LUZ ENTERINA” and “MARILOU ENTERINA” who was, in
fact, born on ““29 December 1960” refer to one and the same mother of ZALDEE
ENTERINA ALVAREZ;

We are not related to her family either by affinity or by consanguinity;

We are executing this Affidavit to attest to the truth of foregoing facts and for
whatever legal purpose this may serve.
