we finding, your diagnosis is Problems in fluid and electrolyte balance which is related to the too
much you lose the water. As we know you already vomitted more that 3 times and loses appetite,
and you also have a diarhea 5 times this day. But you don’t have to worry, we will do some interventions
to improve you health.

Nurse :. So I am going to do some action so we can replace the lost of water.

Patient : what will you gonna do ms?

Nurse : first, im going to Assess your dehydration status, your eye, your skin , its dry or
not, so I can know how much your body loss of water.
Patient : ok ms
Nurse : and then I will asses your intake and output of water, So we can know the water
balance on your body
patient : how are you gonna do that ms?
Nurse : I will monitor how much you drink water, your urine, the fluid on your iv line.
So we will know how much the water balance on your body

Nurse : and then I would like to measure your vital sign every 3 hours. I will measure
your blood preasure, respiratory rate, temperature, and your pulses. So we can know the
effectiveness of the plan

Nurse : and the we will do some laboratory test to determine the need for therapy change
and effectiveness.
Patient : what kind of laboratory test ns?
Nurse : we’re gonna do electrolyte, Hb, and albumin test. So its like some thing that
show much the effectiveness of the the plan ms. So your blood will be taken to be checked into
the lab ms.
Patient : oh okay
Nurse : and the last, I will Collaborate with medical team to prescribing your diarrhea
medicine. So it will help the loss of water in your body.
