AFFIDAVIT

I, LETICIA S. PUNZALAN, of legal age, Filipino citizen, with

residence and postal address at Concepcion Grande, Naga City,
Camarines Sur, after having been duly sworn according to law, hereby
depose and say:

1. That I am the incumbent Punong Barangay of Brgy.

Concepcion Grande, Naga City, Camarines Sur;

2. That sometime in the year 2014 the Narra Tree located

inside the Concepcion Grande Elementary School, Concepcion Grande,
Naga City, was uprooted as a consequence of typhoon “GLENDA”;

3. That I have personal knowledge that the fallen Narra Tree

described above were sawn into flitches;

4. That I am executing this affidavit to attest to truth of the

foregoing statements for whatever legal purpose it may serve.
