AFFIDAVIT
I, AIMEE NAMIA BALIN, of legal age, Filipino, married, resident of Blk. 21,
Lot 19, Cisterian St., Camella Homes, Del Rosario, Naga City, under oath, hereby
state:

1. That I am the owner and proprietor of Sweet Velvets

Homebaked Goodies, a business engaged in the baking and selling of
cakes and pastries, which is duly registered with the Department of Trade
and Industry;

2. That the average monthly income I earn from my business is

P20,000 to P30,000;

3. That I am executing this Affidavit to attest to the truth and

veracity of the foregoing facts and to use this as my supporting document
for submission to the Home Development Mutual Fund (Pag-ibig);
