-: Definition

Archer (1975) proposed that an impacted tooth or an embedded tooth is the


tooth that has failed to erupt completely to its correct normal position in the dental

arch, and its eruption potential has been lost.

Investigation of maxillary impacted canine

clinical signs :-

1. Presence of distinct bulge in the palate or on the buccal aspect of the maxilla.
2. If canine is labially impacted, there will be a deflection of the lateral incisor
(apical portion of root lingually and the crown labially).

Radiographic localization :-

The position of the upper canine is assessed from


Radiographs which are taken in two planes to give
3-dimensional impressions of the tooth and they are :-
1- Intraoral periapical films. 2-Vertex occlusal films

These are
supplemented with
i. Oblique occlusal films
ii. Anterior occlusal films
iii. True lateral films
iv. Tangential views films
v. CBCT

Classification of impacted maxillary canine

Class I :- Palatally Impacted canine ( horizontal ,vertical,semivertical)


Class II :- Buccally impacted canine ( Horizontal ,vertical ,
semivertical( Class III :- Impacted canine located in both the palatal and
labial sides
Class IV :- Impacted canine located in the alveolar process.
Class V :- Impacted canine located in an edentulous maxilla.

FACTORS DETERMINING TREATMENT

a. Age of the patient


b. Stage of tooth development
c. Position of impacted tooth
d. Evidence of root resorption of adjacent permanent teeth

TREATMENT OPTIONS

i. No treatment
ii. Surgical removal of un erupted canine
iii. Surgical exposure of the crown with or orthodontic treatment

- Surgical procedure:

In general the surgical procedure can be summarized in five basic steps:


1. Gaining access by flap elevation.
2. Removal of overlying bone.
3. Sectioning of tooth.
4. Delivery of the tooth with elevators.
5. Preparation for Wound closure.

- based on the data from the clinical and radiographic examination, the
Surgical removal of maxillary impacted canines may be per-formed in
three ways:-

1) the labial approach,


2) the palatal approach
3) combination of the two
