REGISTRATION IS INCOMPLETE WITHOUT SIGNATURE AND MUST BE COMPLETED BEFORE THE EVENT

The Malayan Colleges Laguna does not provide liability insurance for the protection of individuals, groups,
organizations, spectators, or others who may participate in the [ACTIVITY].

I understand and agree that in participating in said [ACTIVITY], there is a possibility of damage, loss or
injury. I voluntarily agree, therefore, to assume all risk and responsibility for any such injury or accident,
which might occur to me or my child during [ACTIVITY].
This release extends and applies to, and also covers and includes, all unknown, unforeseen, unanticipated
and unsuspected injuries, damages, loss and liability and the consequences thereof, as well as those now
disclosed and known to exist. If I am signing this waiver for my children, I certify that I am the parent or
legal guardian and have the right to waive these rights.

Permission is granted to Malayan Colleges Laguna to utilize each Participant’s name, voice, statements,
photograph, images, likeness, actions in any live or recorded footage from the games (including but not
limited to any form of video display, transmission or reproduction), in whole or in part, for promotional,
commercial or similar purpose without any additional consideration; for this purpose, we acknowledge
and agree that MCL and/or its authorized representatives and partners may communicate with us by
electronic mail, text or other form of correspondence.
