    (1) Drivers License, Must have Picture and Signature on it
    (2) Military ID
    (3) User Names and Passwords for IACRA and psiexams websites

    IF YOUR DRIVER’S LICENSE DOES NOT HAVE YOUR CURRENT ADDRESS OR


    THE MILITARY HOME OF RECORD, bring one of the following:

    Lease or Rental Agreement (Signed)


    Mortgage Statement
    Homeowners or Renters Insurance
    Property Deed
    Property Tax Bill or Receipt
    Motor Vehicle Title/Registration Documentation
    Voter Registration Card

    DD Form 93, Record of Emergency Data


    DD Form 2058, State of Legal Residence Certificate
    FAA Form AC 8060-1, FAA Airman Certificate
    FAA Form 8060-4, Temporary Airman Certificate
    FAA Form 8420-2, Medical Certificate and Student Pilot Certificate
    FAA Form 8500-9, Medical Certificate
    FAA Form 8610-1, Mechanics Application for Inspection Authorization
    FAA Form 8610-2, Airman Certificate and/or Rating Application
    FAA Form 8710-1, Airman Certificate and/or Rating Application
    FAA Form 8710-2, Student Pilot Certificate

    Total Cost is $180.00, a $160.00 Payment will be remitted to psiexams.com by credit


    card during registration. An additional $20.00 will be due on the day of the class/exam
    and paid to The Test Center directly. That payment may be made by Master Card,
    Visa, Discover Card, Cash or Check. Credit cards will be charged a user fee of 4%
    over and above the $20.00 charge.
