Dear Chairperson Aquino:

This refers to our request submitted to the MNRC Secretariat on 18 May 2020 for the
issuance by the Microfinance NGO Regulatory Council (MNRC) of the IATF ID or
equivalent Certification to _____________________________________.

Based on the evaluation, we are re-submitting the list of employees incorporating the
following:

1. IATF Accreditation ID No. is indicated for each employee based on the

Company Code assigned to our company;

Also, we are sending CERTIFICATION incorporating conditions stated under the 2nd
paragraph of requirement no. 2 in the Notice published by MNRC on 12 May 2020 (i.e.,
pregnant, among others).

Further, as part of the verification process, attached here is a sample Certificate of

Employment with the employee’s age, residence, office address and other pertinent
information.

On work arrangement for areas under ECQ/MECQ, skeleton forces will be implemented
on a rotational/alternate basis. Employees to be assigned are mostly local residents. On
the other hand, for areas under GCQ/MGCQ, 50% workforce will be implemented
wherein the 50% of employees will be working physically in the office while the other
50% will be doing work from home arrangement. There will be a rotational or alternate
arrangement as well wherein those working from home will be reporting to office
alternately or in rotation with those who have reported to office.

Lastly, for transportation, employees residing in Luzon but assigned in Visayas or

Mindanao, or residing in Mindanao but assigned in Luzon or Visayas, or residing in
Visayas but assigned in Luzon or Mindanao, who have been in their respective home or
residence address during the period when community quarantine was declared, they will
not be allowed to travel yet unless there is a formal confirmation from IATF declaring
normal air transportation operation.

Said employees will be temporarily assigned to their local address/home provinces/

home islands until such time that community quarantine is lifted, and that again a formal
confirmation from IATF declaring normal air transportation operation.

In case air transportation and/or sea transportation is already allowed, all employees, as
per existing company policy, when travelling will be pick-up point to point -- from their
home to the airport and/or sea and once landed, another service vehicle is pre-arranged
to pick them up and bring them to their destination.

Those employees who, during the period of community quarantine, were trapped in their
work of assignment while on duty are being provided with a staff house by the company
where to stay.

A company service vehicle is provided for employees who will be reporting to the office
to minimize exposure to risk. They will be housed in the staff house currently being
provided by the company and is located within the office premises. However, those who
will be returning to their home after office hours, a service vehicle will also be arranged
and provided, thus most of the employees are returning to their home weekly or every
15th or 30th.
