1. That I/WE am/are committed to fulfill the full payment of the tuition fee, miscellaneous and
other fees on or before October 2020 of the inclusive school year; That I/We am/are committed to
adhere to the prescribed schedule corresponding the mode of payment ;

2. That a corresponding flat rate surcharged of 5% (for payment in monthly installment basis) 09759043208

3. That whenever a school activity requires an Activity Fee2 below Php500.00, a notice of

the Activity Fee, I/We undertake to sign the Enrollment Application Form authorizing the Finance (EMAIL ADDRESS)
Department to charge to my /our son/daughter's account the unpaid Activity Fee, which will be settled
in accordance with the provisions in the form.
4. That I/We am/are aware and agreed that an additional Php400.00 monthly charge for the
online fee shall be collected effective June to March.

5. That I/We am/are informed that there will be no release of results of quarterly grade reports
until accounts are settled;

6. That my/our son/daughter/ward will not receive his/her report card during the date of
distribution and he/she may not be able to qualify for recognition of quarterly honors due to incomplete
grades;

7. That I/We am/are made aware that the School reserves the right to withhold the transfer
credentials of any Pupil or Student for reasons of dismissal, expulsion, or NON-PAYMENT OF
FINANCIAL OBLIGATIONS or property responsibility of the pupil or student to the School. THE
CREDENTIALS SHALL ONLY BE RELEASED AS SOON HIS/HER OBLIGATIONS SHALL HAVE
BEEN SETTLED or the penalty of suspension or expulsion lifted; and

8.That the School shall have the option not to readmit my/our son/daughter/ ward for the succeeding
school year until all accounts due for the previous school year are settled.

9. Provisions contained in this Agreement may be changed without prior notice.

10. For purposes of getting in touch with you on matters relative to the above, please indicate

11. Should there be any change in the contact number/s, it is my responsibility to update and
inform the School.

12. That I/We am/are made aware that the School reserves the right to adjust the school fee
as the school deems appropriate in the future.

Our Lady of the Angels Seminary will not be liable if a messages were wrongfully sent to other
person's number/s due to inaccurate or not updated contact numbers/s.3
