AFFIDAVIT OF DISCREPANCY

I, ROLEN BANDIOLA TONGCUA, Filipino, single, of legal age, and a resident of Brgy.
Dalid, Sta. Barbara, Iloilo, Philippines, having been duly sworn in accordance with the law,
hereby depose and state;

1. That I am the mother of patient JOHN CARL TONGCUA SENAPITE, a new born baby,
currently confined at Western Visayas Medical Center, Mandurriao, Iloilo City;
2. On August 7, 2019, I gave birth to said John Carl Tongcua Senapite, and during that time
my sister erroneously recorded his name at the said hospital as “BABY BOY MORALES
TONGCUA”;
3. Given the child that my child was born out of wedlock, my sister opted to name it after
my surname in the hospital records, on the other hand, my sister provided a wrong middle
name erroneously;
4. That the errors was due to inadvertence and not intentional;
5. That later on my child was duly recognized by his father namely Joseph Senapite, thus
we were able to register my child as JOHN CARL TONGCUA SENAPITE;
6. That JOHN CARL TONGCUA SENAPITE as provided for by his Certificate of Live
Birth and BABY BOY MORALES TONGCUA as per his medical records in Western
Visayas Medical Center refer to one and the same person;
7. That I am executing this affidavit of discrepancy to attest the truth of the foregoing
circumstances and for whatever legal purpose that this Affidavit of Discrepancy may
serve;

IN WITNESS WHEREOF, I have hereto set my hands this 19th day of September 2019 in
