I-Human, significant weight loss, 81%

I started the scenario in I-Human with J.J, 19-year-old female who was admitted for

significant weight loss. I achieved a score of 81%. The patient had a history of anorexia nervosa

with hospitalization at 15 y/o for 6 weeks. She took fluoxetine for depression and followed up

with psychotherapy for a year. Recent stressors include moving away from home to college,

classmate telling her she was fat, and not having any close friends. In family history she reports

that her dad is strict and her mom overprotective. Brother used to tease telling her she was hefty.

She began dieting 4 months ago. Her vitals were: BP: 104/60, HR: 56, RR: 14, Temp: 96.8,

Spo2: 98%. Other assessments I found were dry skin, dry and brittle hair, lanugo, exercise daily

(4-5 miles), bradycardia, chills, sad mood, anxious, hypoactive bowel sounds, constipation. I

recommended to follow up in the consult with dietitian. Also, she needs pharmacological

intervention for depression. Non-pharmacological methods like meditation and psychotherapy

were also recommended. Monitor closely after and during meals is important in a patient with

anorexia, as well as offering several small snacks a day. Monitor weight daily. Evaluate coping

mechanisms and resources around college are essential to the maintenance of her wellbeing and

her school work and job activities.


In this case, I deepened my knowledge in anorexia nervosa. Depression was a big factor

in her onset of symptoms again, so to treat the cause and underlining issues is key in her future

recovery and quality of life. My weakness was not completing all the questions suggested in the

interview. My strength in this scenario was identifying the actions that needed to be taken in the

plan of care of the patient putting s priority all the steps related to resolved her imbalances

nutrition.
