AFFIDAVIT

I, Christine Virgeneth U. Fuentes, of legal age, Filipino, single, resident

of IPI COMPPOUND, TALAMBAN CEBU CITY, after having been sworn to in
accordance with law, do hereby depose and say:

1. That I am the registered owner of the business establishment

BERRY BENEFITS FOOD with office address at PIEZZA ELESIA
bldg.., Talamban, Cebu City;

2. That as a registered owner of the said business I was issued by the

Office of the BUREAU OF THE INTERNAL REVENUE (BIR)
Certificate of Registration, Notice to the Public to issue receipt,
however, the Notice to the public to issue receipt document cannot
be found despite diligent effort ;

3. That the undersigned failed to print a receipt due to the fact that the
business ;

4. That upon discovering the forgoing fact, we exerted our best effort
in recovering the said document but the same could not be restored.

5. That I am executing this affidavit in order to attest the fact that the
said owner’s duplicate copy of TCT NO. 41690 was destroyed and
now beyond recovery.
