1. Ask the driver for a printed copy

2. Fill up all required information
3. Submit the completed form back
to the driver

B. ONLINE FORM

1. Go to your Facebook account

2. Search for the “RTA Public
Information and Education”
page
3. Scroll down and look for the post
for the “Contact Tracing Form”
4. Click the link. It will send you to
the Google Form
5. Please fill up all required
information
6. When done, please click
“Submit”
