Commission’s letter No.C-8/8(9}/2018/769 dated 27/30.08.2011 In continuation to above, the checklist to be enclosed with proposals of ROB/RUB/FOB/RF and Railway Bridges has been updated and is forwarded for your information and necessa| action. The checklist duly filled needs to be submitted along with the application for sanction minor works related to ROB/RUB/FOB/RFO and Railway Bridges. ‘* The application for the works of ROB/RUB/RFO/Bridges executed by Railway should He submitted to Commission by CBE. ‘In case the work is being executed by Construction Organisation, the application may be submitted by CE(Con). ‘© In case the work is being executed by PSU or other Department, the application should b} submitted by CBE. ‘* Incase of FOB being executed by Division, the application may be submitted by DRM. Already the above practice is being followed by NER and NCR. Necessary instructions may be given to Division/Construction agcordingly f implementation DA : Revised Checklist. General Whether the GAD has name and designation of the officials with date, who has signed it? t General Whether the application is for commencement of the work or for launching or for both? If it is for launching only, reference of sanction for commencement may be given. General Whether technical note of CBE has been attached for the proposed scheme? Whether GAD has been prepared as per check list issued by RDSO vide letter no. CBS/DBR/IMP/Policy dated 02.11.2017? Para wise compliance of the check list to be submitted. (The check list is to be ensured for all Railway Bridges/FOBs/RUBs/RFOs/ROBs) GAD Whether the GAD has been prepared with key plan showing existing alignment with all features like bridges, curve etc., up to the curve/stations on both side of ROB? (xv) Whether the dimensions of plan, | (xvi) Whether backfill material behind abutments, wing walls, RCC boxes etc. have been shown? (xvi) Whether weep holes have been provided adequately? (x) Whether proper facilities for inspection like inspection ladder, steps, inspection platform have been provided? If not, the reasons there of. If so, whether these have been provided as per bridge manual? | 21 [Railway Bridges |In case of jacketing of abutment / pier, whether the decreased waterway is more than the required waterway? Railway Bridges In case of Railway bridges for doubling/gauge conversion, (i) Whether safety of existing structure and bank has been examined and the Proposed new construction/ extension is safe? (i) Whether hydrological investigation has been carried out? (Note: Hydrological investigation should invariably be carried out for all projects of new line and even for the bridges in doubling, | GC projects) (ii) Whether completion drawings of the existing bridge have been submitted? (iv) Whether there is any inconsistency in the proposed work w.r.t completion drawing of existing bridge? (v) Whether the bridges have been checked and certified for design loading indicated in the drawing? (vi) Whether the alignment of the project has been finalised and approved by open line and a copy of entire alignment of the project has been submitted to commission with this application or with any of the application sent earlier (reference may be given)? (vil) Whether soft copy of the alignment of the project has been submitted to commission? (vii) Whether in case of existing Arch bridge, load testing has been done for the proposed loading to certify its fitness for the proposed loading (Generally 25T)? (ix) Whether the load of the new line is coming on the existing bridge? If so whether the existing bridge has been | certified for proposed new loading } (generally 251)? ROB/RUB/RFO/ | In case temporary arrangement is proposed, Railway Bridges | whether the detailed design and drawing has been submitted by the executing agency to CBE and have been checked and certified b | cBE? 28 Launching | scheme |Whether launching scheme has been approved by CBE? (As per correction slip no 36 dated 27-03-2018 to IRBM the launching scheme of all ROB/RUB/Railway Bridges! FOBsIRFOs etc are required to be approved by CBE. CBE while approving launching scheme shall ensure all items of the check list and certify) 29 Launching scheme Whether the launching scheme has been checked and approved? (While approving the scheme all aspects like design, method statements, risk assessment, safety, QAP, stand-by arrangements, restoration in case of emergency, arrangement of mock trials on approaches etc should be called by CBE from the executing agency and checked) 30 Launching scheme @ il) ii) (iv) (Note: Since the work generally involves multi-stage operations, a general method statement needs to be developed by the executing agency which can be further | developed to more detailed and specific method statement. These statements will invariably require diagrammatic as well as descriptive notes covering the erection sequence for each span and highlighting the stress required at all stages, certified by the independent checking authority like proof consultant & IIT. CBE should insist for method statement on the above lines before approving the launching scheme. A risk assessment analysis is required to be done by the executing agency and all measures needs to be taken to ensure safe launching) Whether the method statement has been checked and verified by CBE and whether it includes: Risk assessment and all measurers for avoiding or mitigating the hazards identified in the risk assessment has been made. Step by step procedures supplemented with diagrammatic illustrations. Safety procedures and instructions Procedures for avoiding hazards to personnel working adjacent to the operations. (v) Method to ensure all necessary test and [ examination certificates. | (vi)® Operational and maintenance | manual(s) and any other relevant documents. (vii) Checking and confirmation of the strength and stability during and after erection. (vill) Provision of properly trained and competent personnel. (ix) Means to observe for any unsafe conditions. (x) Contingency plans and procedures to be followed in case of emergency situations assessed in risk assessment (xi) Method for replacement of bearing after their service life. (xii) Material statement with source and make of equipment with test certificates has been submitted and are of standard make and proven service. The critical materials like welding equipment, certified welders, rollers, bearings pullies , winch ete. 31 | ROB/RFO/RUBs/ | Whether the designs of substructure and Railway superstructure have been checked and Bridges/FOBs —_| certified? (CBE should call Design basis, with non-RDSO | design calculation duly proof checked by IIT spans as required before certification. (g) Whether WPS and WPQR has been approved by competent authority? 10 35 Launching Scheme Whether the strength and stability of girders have been checked for stresses during and after erection at every stage? 36 Launching Scheme Whether provision for 100% reserve machinery tools and plants has been made to cater for failure during the launching? 37 Launching ‘Scheme Whether required medical team has been ensured at site to cater any emergency for working personnel? Whether all Safety, Health and Environment | protection plan has been submitted and required measures have been ensured? 38 Launching Scheme Whether the launching scheme has detailed descriptive note about sequence of operation, requirement of speed restriction, traffic and power blocks etc? Whether the scheme requires road/rail traffic diversion plan during construction of ROB/RFO? If so, whether the same has been prepared and examined and bears the approval of competent authority? 39 | Launching Scheme 11 Whether there is any inconsistency in the GAD, launching scheme, modus operandi etc? (CBE to examine that in case of any change of scheme at later stage, suitable amendment has been issued to GAD or any other relevant approved drawing). 40 Launching Scheme Whether the crane working arrangement clearly indicates the location of placement of crane, members to be tackled, boom length and working radius? Whether the load chart of crane proposed to be used, the weight of members to be lifted etc. and specific compliance of Railway Board instructions regarding crane working near track has been ensured? 41 Launching Scheme In case the launching work is proposed to be done in stages, whether the arrangement for securing of girders during all intermediate stages has been clearly indicated? The scheme for final work viz. fixing bearing, removal of temporary works as required etc. shall also be indicated. ry Launching Scheme Whether manufacturer's guarantee certificate of individual component like bearings has been ensured wherever relevant? 43 Launching Scheme Whether the QAP for fabrication and erection of girders and after erection of girders for check on camber etc. has been submitted and verified? 44 Launching scheme | name and designation responsible for quality Indicate team of the railway officials with control and inspection of critical components viz. welding, bearing ete. 45 Launching Scheme Whether launching scheme is as per the RDSO standard launching scheme wherever applicable? 46 Launching Scheme Whether RDSO guidelines has been followed for fabrication of girders?
