MEMORANDUM

FOR : Chief, AHO

SUBJECT : Explanation

DATE : June 11, 2020

1. References:
a. Memo from Chief, AHO dated June 11, 2020 with subject same as
above;
b. Attendance Sheet re: Accounting of Detailed Personnel dated June 11,
2020 (5AM); and
c. Presidential Proclamation NO. 922 dated March 8, 2020 “Declaring a
State of Public Health Emergency through the Philippines.

2. Above references pertains to the conduct of Accounting of Personnel re:

Enhance Community Quarantine in Metro Manila at the NCRPO Grandstand, Camp
Bagong Diwa, Bicutan, Taguig City last June 11, 2020 wherein the undersigned was
marked absent.

3. Please be informed that while it is true that the undersigned personnel failed
to attend the Accounting of Personnel on the questioned date, his inability to attend the
said activity was brought by the inclement weather Tropical Depression Butchoy where
it affected the Central Luzon making roads from Brgy Malinta, Valenzuela City to
Bicutan, Taguig City unpassable.

4. However, despite of his inability to attend the Accounting of Personnel at the

NCRPO Grandstand, he reported in his Area of Deployment located at Quirino Highway
near Alat Bridge, the boundary of Caloocan and Bulacan. To prove his presence and
rendition of duty, attached herein is the Inspection Report and Daily Attendance dated
June 8, 2020 herein attached (ANNEX “1”) conducted by PLT FERNANDO C QUIZON
where his name and signature are shown. He was also inspected by the RFMB in its
Accounting and Inspection Report dated June 11, 2020 proof of which is herein
attached (ANNEX “2”) and photographs taken at his post on the date in question.
