PARENTAL CONSENT

_________________________ has my permission to participate in the Annual

Regional Convention this coming February 21-23, 2020 in General Santos
City, South Cotabato. I agree that this event broadens the students’ skills and abilities
in their chosen field through the academic activities in the event. Also, it allows them to
develop camaraderie and sportsmanship through teamwork and social interaction in
various non-academic activities in the convention. This facilitates a more meaningful
experience in addition to the regular classroom instructional programs.

Inherent risks are associated with any activity. However, I believe that the opportunity
for learning and awareness outweighs these risks and I hereby grant permission for my
son/daughter to participate.
