WAIVER

2. As the daughter of the late [name], I understand that I am interested in his estate – which

3. Nevertheless, I agree that my sister, [name], be substituted in the stead of my father in the
aforesaid account. With full comprehension of the potential consequences of this action,
she may now hold the aforesaid account jointly with my mother, [name of mother];

4. I hereby acknowledge that I have carefully read this Waiver, understand the contents
thereof, and am executing it voluntarily of my own free well.
