CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT OF SERVICE made and executed by and between:

residence at _________________________, hereinafter referred to as the
FIRST PARTY;

_________________________, of legal age, Filipino, married with

residence at _________________________, Philippines, hereinafter called the
SECOND PARTY.

WITNESSETH:

WHEREAS the FIRST PARTY needs to hire the services of the SECOND

PARTY is willing to pay the sum of

ACKNOWLEDGMENT
