Rates of Cultures 1.1. Since February 9, 2015, the PNCM is releasing most of its cultures in ampoules (Liquid-dried/preserved form) unless otherwise requested. Some cultures held in the PNCM are not preserved in liquid-dried form, i.e. non-spore forming multicellular fungi, hence, may only be released in active form. Active culture rates will apply to these requests. 2. Requirements for requesting cultures 2.1. A letter of request addressed to Dr. Rosario G. Monsalud (Director, BIOTECH). Indicate in the letter: 2.1.1, the microorganismis requested, 2.1.2. the purpose for requesting the microorganism/s, 2.1.3. the customer's CAPABILITY to handle the organism, i.e. if laboratory to be used is properly ‘equipped with a Class 2 biohazard safety cabinet particularly for biosafety level II organisms such as Salmonella, if cultures will be handled by a microbiologist, and/or if will be used under the supervision of =a trained microbiologist. FOR HIGH SCHOOL STUDENTS, COLLEGE STUDENTS AND GRADUATE STUDENTS WHO HAVE NOT BEEN TRAINED IN MICROBIAL HANDLING AND DISPOSAL TECHQNIQUES, the letter of request should be written and signed by the student's adviser (or a highly trained individual who is ultimately responsible for supervising the student's research) on the student's behalf indicating everything mentioned in 2.1.1, 2.1.2, and 2.1.3. The letter adviser should be noted by the person with the next higher rank e.g. Department DOCUMENT Director, Principal and etc. STUDENTS should not write the letter. Students as mentioned in 2.1.4 must let their adviser (or a highly trained individual who is ultimately responsible for supervising the student's research) fill out and sign the said form. Students as mentioned in 2.1.4 must let their adviser and a highly trained individual who is ultimately responsible for supervising the student's research fill out and sign the said form. The adviser should fill out both the “Authorized Representative" and “Scientist” fields in the absence of other supervising individuals. However, if @ highly trained individual will be supervising the student, e.g. a DOST Microbiologist, the said individual should fill out the “Scientist” field and the adviser should fill out the “Authorized Representative" field. 2.4. The full payment for the cultures. A customer can pay in person or it can be sent via LBC Pesopak (Door-to-Door Delivery). Cheques are also accepted, provided they are payable to BIOTECH. Personal cheques are not allowed. 2 The courierishipping fee (Optional). A customer may choose to pick up the cultures in person once they are ready or the customer may add a courier fee to your payment if the customer wishes us fo send his or her cultures. For Luzon and Metro Manila areas the fee is PhP 170.00 while for the Visayas and ‘Mindanao areas the fee is PhP 185.00 for every ten cultures (in tubes). The courier fee should not be included in the cheque, but must be sent via LBC Pesopak. Official receipts will be sent together with the culture/s. 3. Mailing Address of Requirements 3.1. For customers who cannot come in person they may send their requirements to the PNCM, 3.2. Monsalud, University of the Philippines Los Banos, College, Laguna 4031. 4. Processing of Requests, Culture Preparation, and Culture Release 4.1. The processing of a customer's request shall commence only upon receipt of al the requirements. 4.2. L-dried (preserved) cultures may be released on the day the requirements are received but tis will depend on the availabilty of the stocks. If there are no stocks available on the day the PNCM receives all the documents then the customer will be given a lead time like that when requesting for active cultures. for mixed requests, e.g. L-dried cultures with active cultures, cultures will be released active culture is prepared. However, L-dried cultures may be released ahead of the UNCORTROMEEE =! that the customer provides the necessary courier fee (for non pick-up requests).
