Re: Ms Dolores Haffmann, DOB: 22.06.1986

I am writing to refer Ms Haffman, a sales assistant, whose features are consistent with
reactive depression and anxiety, for your further evaluation and management.

On 22.11.2014, Ms Haffman presented with the complaints of major symptoms of


depression following stress at the workplace. In addition, she started living alone after being
separated from her boyfriend and wishing for a break from work. She was also thinking of
quitting her job. Thus, referral to a psychiatrist was offered; however, she denied it. Besides,
temazepam was prescribed. After a week, she was found non-compliant to medicine and
agreed to see a psychiatrist.

Initially, Ms Haffman attended in December last year, when she exhibited an unconscious
state for 5 to 10 minutes following an alcohol overuse. Subsequently, she recovered
uneventfully. After this occasion, she visited multiple times with different medical issues and
was treated accordingly. She had been investigated with a series of tests including a
Paulbunnel test which revealed unremarkable.

Ms Haffmann is a smoker and a drinker. She is allergic to penicillin.
