Focus: Impaired Verbal Communication related sensory challenge

Data: Received lying on bed with an IVF of PNSS 1 liter x KVO @ 800cc level; “Hindi
ko maigalaw minsan bunganga ko’ as verbalized by the patient; with slurred speech;
With an O2 at 2LPM via NC;With intact and patent NGT; With intact and patent IFC
connected and draining in a urine bag ;With clean and dry adult diaper.

Action: Assessed overall status; Assessed psychological response to communication


impairment, willingness to find alternate means of communication. Monitored vital signs;
Evaluated the patient’s energy level. Learned patient needs and pay attention to
nonverbal cues; Placed important objects within reach; Maintained eye contact with
patient when speaking. Stand close, within patient’s line of vision; Had given the patient
ample time to respond; Maintained a calm, unhurried manner. Provided sufficient time
for patient to respond. Provided environmental stimuli as needed.

Response: able to use a form of communication to get needs met and to relate effectively
with persons and his or her environment.
