Dear Mr. Gonzaga,

We would like to express of our firm’s sympathy for the unfortunate incident that happened to you and
your family.

Before we move forward with our actions and decisions regarding the incident, our firm would
like to as ksome questions to you, for clarificatory and informative purposes:

Personal Circumstances:
1. Please state your name and your personal circumstances
2. Why did you choose Bunales, Gomez, Pascual, Pertierra, Ricafort and Associates as your law firm
of choice?
3. Have you seen or consulted with another law firm or lawyer regarding the incident for the past
four (4) months? If yes, please state his/her/the Law Firm’s name
4. Given your age and the circumstances affecting the incident, how are you feeling and coping
right now?
5. Aside from being a senior citizen, do you have other illnesses? What are these, if any (whether
psychological or physical)?
6. What were the medicines for?
7. Are you a person with disability (PWD)?
8. What is your current standing in your community?
9. Has Mr. Samaniego attempted to speak to you since the incident?
10. Is there any occasion where Mr. Samaniego has displayed reckless and wanton behavior (maybe
towards another neighbor)?
11. How much did you spend for you and your wife’s counselling?

Questions regarding the Incident

1. Do you have a Certified True Copy of the Subdivision’s strict rules against street parking?
2. Have you reported the behavior of Mr. Samaniego to the United Homes Subdivisions
Homeowners’ Association?
3. Have you gone to your Barangay hall for a possible barangay conciliation
4. Do you have any actual costs of the damages incurred by your vehicle (specific cost per damage,
that amounted to Php80,000.00)?
5. Did you acquire any physical injury?
a. Do you have a Medical certificate to support this?
6. Did you acquire any psychological trauma during the incident?
7. Do you have a CCTV installed in front of your house or anywhere in your house that may show
that Mr. Samaniego parked his car outside his house

them for an interview as possible witnesses)

garage, and bought the medicines you needed?
9. Who were the neighbors that helped Mr. Samaniego back inside his house?
10. During the incident, who were the persons who personally witnessed the incident?
11. Can they give their sworn statements?

be able to determine the exact steps that need to be taken as we progress with your case.
