Unfolding Patient’s Stories

Rashid Ahmed, a 50-year-old male, is admitted to the medical unit with bacterial
gastroenteritis. Upon arrival at the unit, the nurse begins the admission assessment. What
interviewing techniques can be used by the nurse to establish trust and obtain accurate and
thorough information for planning his care?

The nurse needs to use the Five step Model because the goal of this model is patient-
centered interviewing this model can establish trust and obtain accurate information. The first
stage is to set the stage for the interview. The nurse makes sure that the patient is in a
comfortable position. Second, elicit the chief complaint and tell the purpose why you are asking.
Third stage, open the history of the present and ask the patient if he experienced it before.
Fourth stage, continuing the patient-centered history of present illness the nurse should explore
the patient's description of symptoms, emotional or social context of symptoms. The last stage,
transition to the clinician centered process summarizes everything the patient’s answer to
confirm accuracy of information then informs the patient that the nurse will ask several specific
medical questions about the symptoms to the patient.

Part 2 Rashid Ahmed, who is admitted to the medical unit with bacterial gastroenteritis. During
the admission assessment he informs the nurse that he is a Muslim. What questions can the
nurse ask to identify his religious and spiritual needs during his hospitalization? How can the
nurse incorporate his Muslim practices into the plan of care?

issues, and the unacceptable medines.
