Hydroxyurea (HU): 30 mg/kg/dose PO daily repeated every week. Record actual dose given in milligrams.
Cytarabine (CYT): 300 mg/m2 IV on day 1 repeated every week. Record actual dose given.
Monitor: CBC, Lansky, Toxicity reporting ever week, LFT, weight every other week. Record WBC in
thousands i.e 8,500 as 8.5, Hb in gram/dl, PLT in thousands i.e. 345,000 as 345.
Toxicity: See back, code toxicity i.e. H-1 for grade 1 hematologic toxicity
