HISTORY OF PRESENT ILLNESS:

Four hours before admission, the patient


complains of epigastric accompanied by two episodes of vomiting of
approximately 50 ml of vomitus. Patient medicated with a Maalox
tablet with no relief. An hour before admission, abdominal pain
localizes to right lower quadrant, which increase in severity, which
prompted the patient sought admission. The patient was then admitted
to the surgical ward.

III. PAST PATIENT’S HISTORY:


The patient is a known hypertensive but is not
fully complying with his maintenance medications. The patient claims
only to consume alcoholic beverages during special occasions but
consumes one pack of cigarettes in a day for 20 years.
 Childhood Illness: No known series of childhood illness.
 Accidents or Injuries: Never had experience an accident
