PROCEDURES UPON GETTING THE MODULES AND PASSING THE
OUTPUTS OR ACCOMPLISHED WORKSHEETS OF THE STUDENT
1. Parents must present their school pass upon entering the school’s
vicinity. ( Issued school pass has color code refer to the given schedule
of each color.)
2. Everyone should wear a face mask.
3. Disinfect footwear through footbath and observe social distancing.
4. Wash your hands with soap and water at a hand washing sink.
5. Fill-in the attendance sheet on the information area.
6. Everyone is subject to mandatory temperature check, so let the guard
check your temperature.
7. After the temperature testing, you may proceed to the designated
room for getting and returning of modules.
8. The adviser will look for the output of your child/student.
9. The adviser will put a check mark in the output form of your
child/student.
10.The adviser will also check the name of your child/student in the
module distribution form.
11. The parent may get the given modules right away.
12. The modules will be taken by the parent before he/she leaves.
13. The parents will proceed to the exit gate.

Makita sa school pass ang skedyul sa pagpasa sa output og
pagkuha sa modules.
2. Kinahanglan magsuot ug facemask adisir mosulod sa
eskwelahan.
3. Kinahanglan magfoothbath pagsulod para sa disinfection ug
dapat masunod ang social distancing.
4. Maghugas dayon ug tarong sa mga kamot gamit ang tubig ug
sabon sa may hand washing sink.
5. Mag fill ap sa mga kinahanglan na impormasyon sa may
information area.
6. Kinahanglan na makuhaan ka sa guard ug temperature gamit
ang thermal scanner.
7. Paghuman, moproceed ka diritso sa designated na room para
magpasa og outputs o worksheets sa imong anak og magkuha
og modules.
8. Pag magpasa nakag output o worksheets sa imong anak,
pangitaon sa adviser ang Output Form.
9. Tsikan sa adviser ang pangalan sa imong anak sa Output Form.
10. Sa magkuha nka og module, tsikan usab sa adviser ang pangalan
sa imong anak didto sa Module Distribution Form.
11. Human matsikan ang pangalan sa imong anak, pwede na ikaw
mokuha og modules.
12. Pagmakakuha nakag modules, pwede na ikaw moadto sa exit
gate sa eskwelahan og mouli na diritso sa inyong balay.
