The preliminary hearing of our case originally scheduled on November 20 2008 is moved on 27
November 2008 at 9:00 in the morning.

Please bring the following documents with you on the scheduled hearing:

(a) Marriage Certificate (spouses)

(b) Proof of Financial Capacity
(1) Bank Statement or Certificate of Time Deposit
(2) Certificate of Employment
(3) Title/Tax Declarations of Real Properties
(4) Motor vehicle Registration
(c) Medical Certificate of the Spouses
(d) Medical Certificate of the Child
(e) Certificate of Good Moral Character (Please get the certificate from the Barangay Captain)
(f) Certificate of No Criminal Record from the following agencies:
(1) Municipal Trial Court of Dalaguete
(2) Regional Trial Court-Argao
(3) NBI Clearance
(4) Prosecutor Clearance (Get this Certificate from Prosecutor Noel Lawas- RTC Argao

Atty. Ana Marie P. Militante will be the one to appear on the first hearing because I will be out of the
country for three (3) weeks starting on Saturday 15 November 2008. On the next and succeeding
hearings, I will be the one to appear.
