PETITION

The undersigned petitioner, unto this Honorable Court respectfully states that:

1. He is a Filipino citizen, of legal age, married, and a resident of Uy Drive, Lumiyap Road,
Divisoria, Zamboanga City;

2. He is a member of the Philippine Bar having been formally admitted on June 4, 2018, a
copy of the Certificate of Admission is hereto attached as Annex “A”;

3. He is a bona fide member of the Integrated Bar of the Philippines and has paid her
membership due on January 7, 2019 as evidenced by the IBP Official Receipt No.
097902 and Certification, hereto attached as Annexes “B” to “B-1”;

4. He has paid the 2019 Professional Tax under Receipt No. 1996589 dated October
10,2019, machine copy of which is hereto attached as Annex “C”;

5. As a Notary Public he holds office at the Hearing Office, Bureau of Fire Protection
Regional Office 9, Mayor Cesar Climaco Avenue, Zamboanga City;

6. Pursuant to Rule III, Section 1 (4) of the Rules on Notarial Practice of 2004, the
following additional requirements are hereto attached as follows:

a. Certification from the Office of the Bar Confidant, Supreme Court, Manila as
Annex “D”;

b. Certification from the Integrated Bar of the Philippines National Office as Annex
“E”;
 c. Certificate of Good Moral Character signed by the two (2) executive officers of
the local chapter as Annex “F”;

d. Three (3) pieces photograph duly signed by the petitioner and three (3) specimen
of petitioner’s official signatures, as Annexes “G” and Annex “H”;

e. Clearance from the National Bureau of Investigation as Annex “I”;

f. Official Receipt No. _______________ in the amount of Php 2, 020.00 as proof

of payment of the required filing fee of the Petition as Annex “J”.

PRAYER
WHEREFORE,respectfully prayed before this Honorable Court that peititoner’s
Notarial Commission as Notary Public in Zamboanga City be approved.

Other reliefs just and equitable are likewise prayed for.
