2.2 Financial Statement using the recommended format
2.3 Student Organization Manual
2.4 All presidents/governors of student organizations are automatically blocked at the end of
the semester until compliance with the requirements.
2.5 Other officers/students can be blocked as endorsed by the presidents/governors through
the DSA office.

3. Requirements for Recognition/Reaccreditation of


Campus Organization
3.1. Formal letter of application provided by the DSA office
3.2. Accomplishment Report (documentation and financial statement) (current year)
3.3. Calendar of activities for the coming school year
3.4. List of new set of officers, adviser, and members
3.5. Amendments to the constitution and by-laws, if any;
