Hi JD Community! FOR YOUR INFORMATION. This is a requirement for enrollement for Term
2 ). Thank you!

All GRADUATE STUDENTS are reminded to undergo the periodic health examination to be
conducted by the Health Services Office (HSO) from 1 July to 19 July 2014, Monday to Saturday
from 12:00 noon to 9:00 pm. The medical examination is one of the clearance requirements for
enrollment in Term 2, AY 2014-2015.

Students who are unable to avail of the on-campus facility should visit the HSO to ask for a referral
slip in order to undergo the periodic health examination in one of the accredited medical clinics.
Corresponding fees will be charged by the accredited clinics. Students should submit medical and
dental forms filled out by accredited clinic personnel to the HSO.
